Adventure Hook

A local festival in the fields surrounding Ethimer turned tragic when a newly debuted ale plunged dozens of visitors into an inescapable coma. The victims now lie motionless on row upon row of cots inside a nearby civic hall, cared for around the clock by distraught family members. The party must investigate the nature of the brew, uncover how to cross into the victims’ minds, and navigate a shared dreamscape to free them.

Encounter Tier: CR 5 (1,800 XP) | Investigation, Roleplay & Exploration

The Trigger: Triggers upon visiting the civic infirmary or festival grounds near Ethimer. The party finds rows of cots holding comatose victims who drank the new ale. Family members tend to them as the victims toss, turn, and murmur in total synchronicity.
The Objective: Investigate the afflicted villagers and festival leftovers to discover that the victims’ minds are tethered inside a shared dream realm. To break the trance, the party must cross into this domain and shatter the illusion trapping them before time runs out.

Adventure Progression

Time Limit: 7 days after the trigger event. Every passing day causes the comatose victims to gain 1 level of exhaustion from psychic strain and the central dream manifestation grows stronger (+10 temporary HP per day delayed).

  • Phase 1 — Responding to the Crisis: The party must weigh their approach:
    • Investigate Immediately: The party dives straight into analyzing the villagers and kegs, preserving the full 7-day timeline and minimizing civilian exhaustion.
    • Seek External Assistance: The party may consult local druids or herbalists before crossing over. Doing so grants the party Advantage on Wisdom saving throws inside the dream realm, but gathering the rare protective ingredients they recommend consumes at least 1 full day of the time limit.
  • Phase 2 — Investigating the Afflicted: The party examines the comatose victims and festival kegs to uncover the trap and learn how to cross over safely.

    DM Note: The central boss inherently emanates a minor Intoxicating Haze Aura (10 ft radius; DC 12 Con save against Poisoned). The party can attempt up to 3 investigation checks below. Each individual check reveals unique lore on a success, while total successful checks unlock cumulative tactical benefits:

    • 1 Total Success: Reveals the party enters the dream realm by heating the residue to inhale a controlled vapor dose.
    • 2 Total Successes: Reveals the dreamscape’s layout through a momentary hallucinatory vision.
    • 3 Total Successes: Completely neutralizes the boss’s Intoxicating Haze Aura.
    • NPC Assistance: If all checks are failed, the DM may introduce a local official deciphering a burnt brewing journal which grants ONLY the 1 Total Success result at the cost of 2 full days of the time limit.
    • First Check (DC 14 Wisdom/Insight or Arcana): Observing the victims reveals that their movements, flinches, and mumblings occur in total synchronicity, exposing a subtle psychic pulling sensation drawing all their minds toward a single focal point.
    • Second Check (DC 15 Intelligence/Investigation, Nature, or Alchemist’s Supplies): Inspecting the kegs successfully reveals an iridescent, non-evaporating residue inside. Analysis identifies planar leakage, revealing that the ale acts as an anchor binding drinkers’ souls to a pocket demi-plane.
    • Third Check (DC 14 Arcana or Detect Thoughts / Identify): Probing a victim’s mind or the residue’s magic directly locates the tether’s pulse, revealing that a parasitic entity is actively feeding on trapped minds. On a failure, the caster takes 2d6 psychic damage from mental backlash.
    • Alternative — Direct Ingestion: Drinking the ale bypasses remaining checks and plunges the PC into a comatose, Charmed state in Phase 3. Awake allies must still find an entry method. (Charmed PCs view the dream as real; personal instincts, skepticism, or friendly brawls trigger Phase 4 conditions).
  • Phase 3 — Navigating the Dream Realm: Once a character or NPC enters the dream realm, their minds project into a surreal, golden-hued reflection of Ethimer’s surrounding countryside. Sky made of amber foam drifts over fields of giant, towering hop vines. All afflicted villagers or characters are wandering a perpetual, joyous festival inside the dream. Because the dreamscape grants them total euphoria, they do not want to wake up and will actively resist attempts to rescue them.
  • Phase 4 — Waking the Dreamers: To free any trapped mind (villagers or player characters), the party must shatter the dream’s euphoric hold. These methods ONLY work inside the dream realm. No attempts to wake them in the material plane will rouse a comatose body. Players inside the dream can use roleplay, magic, or force to break a dreamer free using these categories:
    • Psychological & Personal Truths: Invoking real-world memories, confronting a dreamer with glaring logical flaws, or presenting a meaningful personal object forces a DC 15 Charisma (Persuasion) check to shatter the illusion through reason.
    • Arcane & Spiritual Disruption: Targeted spells that suppress or cleanse mind-affecting magic (e.g., Dispel Magic, Calm Emotions) instantly sever the trance. Alternatively, a spellcaster can attempt a DC 15 Intelligence (Arcana) check to directly unspool the tether’s magic.
    • Physical Shock: Dealing any damage to a dreamer’s projected form inside the dream forces an instant DC 12 Wisdom saving throw to shock their consciousness awake. While effective, this traumatic jolt inflicts 1 level of physical exhaustion on their waking body in the material realm.

    Severing the Tether: Waking a player character restores their autonomy, but does not count toward freeing the villagers. Once 3 villagers are broken free, the pocket realm loses structural stability. The central manifestation, a towering entity of amber foam and hop vines, attacks to defend its remaining feast of psychic energy. Defeating the entity severs the tether entirely, returning all remaining players and victims to the material plane simultaneously.

  • Outcomes:
    • Full Success: If the manifestation is defeated within the 7-day limit and no villagers reach 6 levels of exhaustion (death), all victims wake healthy. The community honors the party with local acclaim, free lodging, and rare brewing ingredients.
    • Partial Victory: If the manifestation is defeated, but one or more villagers die from reaching 6 levels of exhaustion during the ordeal, the remaining victims wake, but the town’s gratitude is muted by the tragic loss of life.
    • Abandonment / Failure: If the 7-day time limit expires without the manifestation being defeated, the victims die as their vital forces are consumed. The manifestation breaks free of the dream realm and physically emerges near Ethimer as a CR 6 regional threat.

Dream-Bound Villagers (CR 0): Use the Commoner stat block from the Monster Manual, pg. 345 (SRD 5.1), enhanced with the following environmental trait:

  • Phantasm: All damage to a villager’s dream form is reduced to 1.
  • Physical Shock: Dealing damage to a villager’s dream form forces an instant DC 12 Wisdom saving throw to shock them awake. While fast, this inflicts 1 level of exhaustion on their waking body in the material realm.

Dream Manifestation (CR 5): To run the central boss encounter at the heart of the dreamscape, use the Otyugh stat block from the Monster Manual, pg. 233 (SRD 5.1), reskinned as an Amber Draught Manifestation. Re-flavor its traits to fit the dream theme:

  • Appearance: A grotesque, pulsing mass of golden froth, wet mash, and thick hop vines.
  • Tentacles: Re-skin as whipping hop vines that grab and restrain characters.
  • Bite: Re-skin as a churning vortex of fermented froth that pulls targets into its alcoholic core.
  • Bite’s Disease/Poison Effect: Re-skin as an overwhelming surge of hallucinatory vertigo and intoxication.
  • Intoxicating Haze Aura: 10 ft radius; DC 12 Con save against Poisoned.

Material Manifestation (CR 6 — Failure Scenario): If the 7-day timer expires without the manifestation being defeated, the entity fully consumes the victims’ vital force and breaks out into Ethimer. Use the Gorgon stat block from the Monster Manual, pg. 171 (SRD 5.1), reskinned as the Draught Abomination:

  • Appearance: A towering, quadrupedal construct of iron keg hoops, splintered oak barrels, wet grain mash, and dripping amber froth.
  • Gore / Trample: Re-skin as massive barrel-bound strikes and crushing waves of fermented sludge.
  • Petrifying Breath: Re-skin as Intoxicating Vapor Breath (DC 13 Constitution save). Victims who fail their saves are stunned and subjected to immediate hallucinatory coma rather than turning to stone.
